提要註:	專科護理師在照護病人過程中，常對於執業安全感到擔憂。自2015年公告預立醫療流程法規後，專科護理師可以依據規定處置病人並開立醫囑，以確保其執業之合法性。然而，過去的文獻多著重於流程內容的書寫及法規的變遷，甚少關注預立醫療流程發展是否符合臨床需求，以及是否提升執業安全感。因此，本研究旨在探討專科護理師使用預立醫療流程後的經驗感受。本研究採質性研究方法，針對南部某醫學中心專科護理師進行研究。研究採用滾雪球方式招募12位研究對象，並以半結構式訪談大綱進行一對一訪談收集資料。資料分析採用內容分析法，包括編碼、歸納及形成主題。研究過程遵循Lincoln & Guba (1985)所提出研究嚴謹度準則，包括真實性、應用性、恆定性及中立性，以確保整體研究品質。研究結果共整理出四個主題：政策支援不足、提升職場新手的執業安全感、確保執業安全及完整性、以及對未來發展的期待。研究發現，專科護理師在使用預立醫療流程後，確實感受到執業安全感的提升，但對於現有醫院的應對政策仍存在疑慮。他們期望能夠發展例如發燒、呼吸喘、腹瀉等各病房通用的共通項目並加快醫囑套裝化的進程。建議設置預立醫療流程小組以協助統籌，從而發揮專科護理師更多的功能，進一步促進醫療品質的提昇。.

520 3 $a Nurse practitioners often express concerns about safety of their practice when caring for patients. Since the announcement of the advanced medical directives regulations in 2015, nurse practitioners have been authorized to manage patients and issue medical orders according to established protocols, ensuring the legality of their practice. However, past literature has primarily focused on documenting the content of these protocols and evolution of related regulations, with little attention paid to whether the development of the advanced medical directives meets clinical needs or enhances the sense of safety among practitioners. This study aims to explore the experiences and perceptions of nurse practitioners after implementing the advanced medical directives. This qualitative study targeted nurse practitioners at a medical center in southern Taiwan. Using snowball sampling method, a total of 12 participants were recruited. Data were collected through one-on-one interviews based on a semi-structured interview guide and analyzed using content analysis. The data were coded, categorized, and themes were generated. The research process adhered to the rigor criteria proposed by Lincoln & Guba (1985), which include credibility, transferability, dependability, and confirmability, to ensure the overall quality of the study. This study identified four main themes: insufficient policy support, enhanced safety for novice practitioners, ensuring practice safety and integrity, and expectations for future development. The findings indicate that nurse practitioners felt an increased sense of safety in their practice after utilizing the advanced medical directives. However, they expressed concerns about the current hospital policies and hoped for the development of standardized protocols for common conditions such as shortness of breath, fever, and diarrhea, and the acceleration of order set standardization. The study suggests establishing a task force for advanced medical directives to facilitate coordination, allowing nurse practitioners to fully utilize their roles and ultimately improve the quality of healthcare..
