研究重點 旋轉肌肌腱病變是肩部最常見的疾病,常造成患者肩部疼痛、功能下降甚至導致嚴重性關節炎。近期的研究顯示發炎反應在對於旋轉肌病變病程中扮演重要角色。目前治療旋轉肌病變藥物非常有限,尋找有效率的藥物是必要的。蘇拉明(Suramin) 是一種 FDA 核准的古老藥物,許多研究發現蘇拉明具有抗凋亡、抗纖維化、抗炎和抗氧化能力。然而,蘇拉明對肌腱細胞和旋轉肌腱病變的影響仍不清楚。 方法或程序 在關節鏡下採集接受旋轉肌腱斷裂修補手術病人之旋轉肌腱斷端組織進行細胞培養,以MMT試驗評估蘇拉明對肌腱細胞的毒性及活性的影響。Ki-67、PCNA表現和細胞計數用於評估蘇拉明處理後的細胞增殖情況。西方點墨法和即時聚合酶連鎖反應用於測量受蘇拉明影響的肌腱蛋白質及基因因子表現量。細胞遷移測定用於測量肌腱細胞受蘇拉明作用後的遷移性。對於評估蘇拉明對發炎反應及巨噬細胞M1極化的影響,用蘇拉明處理THP-1巨噬細胞再誘導 M1 巨噬細胞極化。 結果 本研究發現蘇拉明可促進旋轉肌肌腱細胞存活及細胞增殖速率,並促進肌腱細胞產生較多的膠原蛋白1及3a。蘇拉明促進肌腱細胞分泌促血管生成因子VEGF分泌。蘇拉明增加肌腱細胞遷移能力。蘇拉明顯著降低THP-1在發炎細胞因子的產生及M1巨嗜細胞極化。這些結果顯示蘇拉明可以有效增加旋轉肌肌腱細胞活性並抑制巨噬細胞發炎反應的產生。它可以作為治療旋轉肌腱病變及促進肌腱修復的潛在藥物。.

Background Rotator cuff (RC) tendinopathy is the most common shoulder disease. Currently, there are limited medication treatments for RC tendinopathy. It is essential to find effective medication for RC tendinopathy. Suramin previously used to treat African sleeping sickness possess the capability of anti-apoptoic, anti-fibrotic, anti-inflammation and anti-oxidative stress. However, the effects of suramin on tenocytes and RC tendinopathy need to investigate. Materials and Methods Tenocytes were isolated form injury rotator tendon tissues in patients received arthroscopic rotator cuff tendon repair. MMT assay was used to evaluate the toxicity of suramin to tenocytes. Ki-67, PCNA and cell count were used to evaluate cell proliferation after suramin treatment. Western blot and RT-PCR were used to measure the protein and gene expression of tenogenic factors in injured RC tenocytes treated by suramin. Migration assay was used to measure activity of tenocytes. THP-1 cell cultured with suramin prior to M1 macrophage induction was used to estimate the effect of suramin on anti-inflammatory response and M1 macrophage polarization. Results In this study, suramin significantly increased cell viability and proliferation rate of RC tenocytes. After suramin treatment, the RC tenocytes expressed significantly higher protein and mRNA of COL1A , COL3A and VEGF. In addition, suramin treatment significantly increase the migration ability of rotator cuff remnant tenocytes compared to that without suramin treatment. For the anti-inflammatory effect, suramin effectively decreased inflammatory cytokines release and M1 macrophage polarization in THP-1 cells. Conclusion These results strongly suggest that suramin treatment could enhance the cell activity and proliferation of injured RC tenocytes. Besides, suarmin could diminish the inflammation and M1 macrophage polarization in THP-1 cell. Suramin could serve as a potential medication for treating rotator cuff partial tear or augmenting the healing after RC repair..
