基督教聖經之智慧書導讀 :	輕度行為障礙是一種神經精神症候群，且被認為可能是神經退化性疾病的前驅期表現。血管疾病是老年人常見的身體狀況，且過去有數項研究指出，血管病變可能導致神經精神症狀的出現。然而，血管病變與輕度行為障礙間的關聯性尚不清楚。本研究的目的是探討以結構性頸動脈粥狀硬化做為血管病變的代表，其是否與輕度行為障礙相關聯。本研究於2018年至2021年間進行，共納入578位居住於社區的參與者。在輕度行為障礙的研究診斷準則中，其症狀範圍包括動機減低、情緒失調、衝動控制差、社會行為不適切、及異常思考或知覺；其症狀須於50歲（含）以後開始發生、持續至少六個月（含）以上。輕度行為障礙的表現是透過由照顧者和參與者填寫的輕度行為障礙檢核表進行評估，而具輕度行為障礙者其輕度行為障礙檢核表總分達8分（含）以上。血管病變則利用頸動脈超音波測量三個頸動脈血管區域—總頸動脈、內頸動脈與總頸動脈分岔處—雙側的斑塊和內膜中層厚度以評估結構性頸動脈粥狀硬化。三個頸動脈血管區域雙側內膜中層厚度的平均值定義為頸動脈內膜中層厚度。若具有斑塊或頸動脈內膜中層厚度達最高四分位數者，則被視為具有進階的結構性頸動脈粥狀硬化。在本研究中，參與者的平均年齡為66.03歲，其中女性占57.4%。在由照顧者填寫的輕度行為障礙檢核表中，經排除11位症狀於50歲前開始發生的個案後，在567位個案中有57位個案具輕度行為障礙（10.1%）。根據羅吉斯迴歸分析顯示，在調整年齡、性別、教育程度、身體疾病、菸酒使用有無及認知功能後，進階的結構性頸動脈粥狀硬化與輕度行為障礙風險之間並無顯著相關。然而，在進階的結構性頸動脈粥狀硬化的兩項組成元素中，僅頸動脈內膜中層厚度與輕度行為障礙的風險有顯著相關；經進一步分析，在輕度行為障礙子領域中，頸動脈內膜中層厚度與動機減退、衝動控制障礙及社會行為不適切的風險有顯著相關。此外，在頸動脈內膜中層厚度的組成元素中，僅總頸動脈的內膜中層厚度與輕度行為障礙風險有顯著相關。本研究顯示結構性頸動脈粥狀硬化所代表的血管病變與輕度行為障礙之間存在關聯性；在結構性頸動脈粥狀硬化的兩項組成元素中，頸動脈內膜中層厚度—特別是總頸動脈的內膜中層厚度—與輕度行為障礙的關聯性最強。本研究指出除了神經退化的病程之外，血管病變亦可能與輕度行為障礙的形成有關。未來仍需進一步研究以探討其因果關係。.

520 3 $a Mild behavioral impairment (MBI) is a neuropsychiatric syndrome proposed as a prodromal manifestation of neurodegenerative diseases. Vascular diseases are prevalent among older adults, and previous studies have suggested that vascular pathology may contribute to the emergence of neuropsychiatric symptoms. However, the association between vascular pathology and MBI remains unclear. This study aimed to investigate whether structural carotid atherosclerosis, as a representative marker of vascular pathology, is associated with MBI. Between 2018 and 2021, a total of 578 community-dwelling participants were enrolled. According to the research diagnostic criteria for MBI, eligible symptoms included decreased motivation, affective dysregulation, impulse dyscontrol, social inappropriateness, and abnormal perception or thought content, with onset at or after the age of 50 and persisting for at least six months. MBI status was assessed using the MBI Checklist (MBI-C), completed by informants and participants. Individuals with an MBI-C total score of 8 or higher were classified as having MBI. Structural carotid atherosclerosis was assessed by carotid ultrasound of bilateral carotid arteries, including the common carotid artery (CCA), internal carotid artery (ICA), and bifurcation of common carotid artery (BIF), focusing on the presence of plaques and measurements of intima-media thickness (IMT). The mean IMT across the vascular segments was defined as carotid IMT. Participants were considered to have advanced structural atherosclerosis if they had either carotid plaques or IMT values in the top quartile. The mean age of participants was 66.03 years, and 57.4% were female. After excluding 11 individuals whose symptoms began before age 50, 57 of the remaining 567 participants (10.1%) met criteria for MBI based on informant reports. Logistic regression analyses, adjusted for age, sex, educational level, medical diseases, cigarette and alcohol use ever, and cognitive function, revealed no significant association between advanced structural carotid atherosclerosis and overall MBI risk. However, among the two components of structural atherosclerosis, only carotid IMT was significantly associated with MBI risk. Further analysis showed that elevated carotid IMT was specifically associated with increased risks of decreased motivation, impulse dyscontrol, and social inappropriateness. Additionally, among the carotid IMT subregions, only increased IMT in the CCA was significantly associated with MBI risk. This study demonstrates an association between structural carotid atherosclerosis and MBI, with carotid IMT—particularly in the CCA region—showing the strongest association. These findings suggest that, beyond neurodegenerative processes, vascular pathology may also contribute to the development of MBI. Further research is warranted to elucidate the causal mechanisms underlying this association..
