Background Influenza is a highly contagious disease that can cause severe complications in high-risk populations. Although vaccination is the primary preventive strategy, the high variability of the influenza virus limits vaccine effectiveness. The clinical efficacy of Oseltamivir remains controversial. This study aims to evaluate its impact on clinical outcomes among high-risk patients, in order to strengthen the existing evidence base. Materials and methods This study employed a systematic literature review and meta-analysis approach, with the overall study design adhering to the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines. Literature searches were conducted across three major international databases—PubMed, EMBASE, and the Cochrane Library—with the search period ending on March 25, 2025. Articles were screened and reviewed in full text by researchers based on predefined inclusion and exclusion criteria, and representative observational studies were selected for analysis. The included population primarily comprised high-risk groups for influenza, such as older adults, individuals with diabetes, and children. The study investigated differences in clinical outcomes between those who received Oseltamivir, those with delayed use, and those who did not use Oseltamivir. The primary outcome measures included mortality, hospitalization rate, and intensive care unit (ICU) admission rate. The quality of the included studies was assessed using the Newcastle-Ottawa Quality Assessment Scale (NOS). Meta-analyses were conducted using the online version of Review Manager (RevMan), employing a random-effects model to calculate the risk ratio (RR) and corresponding 95% confidence intervals (CI) as effect size indicators. In addition, heterogeneity was assessed using the I² statistic. For results exhibiting high heterogeneity, sensitivity analyses were performed to verify the robustness of the findings. Results A total of nine observational studies of moderate to high quality were included in this study. The results of the meta-analysis showed that, compared to the control group, the use of Oseltamivir significantly reduced the risk of disease progression events by approximately 24%, with a pooled risk ratio (RR) of 0.76 (95% CI: 0.63–0.92, P = 0.004), indicating a potential clinical protective effect of the drug in high-risk populations. In terms of secondary outcomes, the analysis of hospitalization rates revealed that the Oseltamivir group had a significantly lower risk of hospitalization compared to the control group, with a pooled RR of 0.70 (95% CI: 0.56–0.88, P = 0.002). Subgroup analysis focusing on adult populations also demonstrated that Oseltamivir use was associated with a reduced risk of further disease progression—such as hospitalization, ICU admission, or death—with a pooled RR of 0.81 (95% CI: 0.67–0.99, P = 0.04), supporting its potential protective benefit. Furthermore, administration of Oseltamivir within 48 hours of symptom onset was associated with an even greater reduction in the risk of disease progression compared to non-users, with a pooled RR of 0.66 (95% CI: 0.49–0.89, P = 0.007). Sensitivity analyses supported these findings, further reinforcing the robustness and positive effect of Oseltamivir in reducing the risk of disease progression. Conclusions The findings of this study suggest that Oseltamivir provides significant clinical benefits for high-risk influenza patients, particularly in reducing hospitalization rates..
