Medication non-adherence is a prevalent phenomenon observed among individuals diagnosed with Serious Mental Illness (SMI). Current literature indicates that individuals with mental illness often encounter substantial difficulties adhering to drug regimens. While various treatment options exist for the SMI conditions, approximately 60% of those individuals diagnosed discontinue their treatment regimen within 2 to 3 months. This happens often because of a lack of access to healthcare, the stigma associated with mental illness, the adverse side effects of medication, or a dissatisfaction with medication management. Medication non-adherence continues to pose a significant obstacle, leading to unfavorable treatment results. Medication non-adherence can be ascribed to vulnerability factors encompassing suboptimal cognitive functioning, diminished motivation, and adverse effects of psychotropic medications. Psychopharmacologic treatments aid in the management of mental health conditions. However, in the United States, non-adherence to medical treatments accounts for approximately 125,000 fatalities. Additionally, medical expenses range from $100 to $300 billion annually, exerting significant pressure on the healthcare system. Despite strategies to enhance medication adherence, challenges still persist. According to Dikec (2023), the prescribed approaches face limitations in addressing complex issues such as homelessness, recurrent hospitalizations, and relapses. Consequently, a medication adherence intervention in the form of SMS is used to prompt patients to enhance overall medication adherence behavior. The anticipated number of participants for the project is 20 (n = 20). Inclusion criteria for participation in the project was a diagnosis of bipolar disorder. Being 18 years or older, and currently taking a prescribed psychotropic medication for at least three consecutive months prior project implementation. The DNP project sought to create a medication adherence strategy that utilizes direct short messaging service (SMS) to improve adherence rates to enhance the participants' overall health.
