基督教聖經之智慧書導讀 :	背景：鎖定式骨釘與骨板系統的發展，對於骨折內固定穩定性的提升具有重大貢獻，特別是在骨質疏鬆患者中，其固定效果遠優於傳統拉力螺釘。然而，近十年來臨床文獻與實務經驗皆指出，鎖定系統在以鬆質骨為主的骨骺端（epiphyseal）及幹骺端（metaphyseal）區域仍有相當高的固定失敗率，常見於股骨頸、轉子間與肱骨近端骨折等部位。造成此現象的主因在於鎖定結構整體剛性（construct rigidity）過高，導致骨折端缺乏足夠的微動（micromotion）及壓迫（compression），進而引起延遲癒合或不癒合。雖然動態鎖定螺釘（Dynamic Locking Screw, DLS）與遠端皮質骨鎖定技術（Far Cortical Locking, FCL）等半剛性設計陸續被提出，部分改善了骨折端應力集中與骨痂形成不對稱的問題，但DLS常因整體結構強度下降而受限於非負重區使用，而FCL需鎖入對側皮質骨，因此僅適用於骨幹區（diaphysis），對以鬆質骨為主的區域仍然不理想。現有鎖定系統在剛性控制與壓迫能力之間缺乏平衡，仍無法同時滿足鬆質骨固定所需之「穩定與柔性」雙重條件。目的：本研究旨在開發一系列新型「二段式可調控復位加壓螺釘」，其可搭配鎖定式螺釘頭及鬆質骨螺紋，稱之為Two-Part Locking Cancellous Screw（TP-LCS），亦具有可調復位（adjustable reduction）與可控加壓（controllable compression）之雙重功能，期望能改善鎖定螺釘在鬆質骨區域固定力不足及結構剛性過高的問題。TP-LCS之設計理念結合了鬆質骨螺紋的深度抓持力與鎖定螺釘的角度穩定性，藉由二段式套筒結構（two-part telescopic configuration），在插入後可進行微幅調整與加壓，以達成骨折端的緊密接觸及穩定復位。本研究透過有限元素分析與生物力學測試，評估此新型螺釘在應力分布、抗拔出強度及扭轉特性等面向之力學表現，並與傳統鎖定螺釘進行比較，以驗證其可行性與臨床應用潛力。研究方法：研究方法分為兩大部分。首先，進行有限元素分析（Finite Element Analysis, FEA），建立十組不同接合長度（10%至100%，每10%遞減）的二段式螺釘三維模型，材料設定為Ti6Al4V合金（彈性係數113.8 GPa、泊松比0.342、降伏強度790 MPa），並施加1000 N軸向拔出力與 1 Nm 彎曲力矩以模擬臨床受力狀況，藉此分析最大 von Mises 應力分布與潛在疲勞集中區。其次，依ASTM F543-13標準進行生物力學測試，包含扭轉性質試驗（torsional property test）、驅動扭矩試驗（insertion/removal torque test）及軸向拔出試驗（axial pullout strength test）。試體分為TP-LCS與傳統AO/Synthes 鎖定螺釘各五支，使用模擬人骨（Sawbones, density = 0.32 g/cm³）作為測試材料。所有測試數據均以平均值 ± 標準差表示，統計分析採獨立樣本 t 檢定與Mann–Whitney U檢定交叉驗證，p 值小於0.05為統計學顯著。研究結果：有限元素分析結果顯示，二段式螺釘的應力分布會隨接合長度改變而重新分配，於30%至90%的組合範圍內可達最均勻之應力分布，顯示此設計能有效降低局部疲勞與結構破壞風險；而當接合比例低於 20% 或達 100% 全結合時，應力集中明顯增加，建議應避免使用此兩極端比例。生物力學測試結果顯示，TP-LCS 在插入力矩與軸向拔出強度方面均顯著高於傳統鎖定螺釘（p < 0.05），表示其在鬆質骨中的固定穩定性與骨內咬合力更佳；雖其扭轉強度略低於對照組（約為其 80%），合理推測是因其芯徑較對照組小所導致，但仍符合足夠安全標準，且具良好延展性與非脆性破壞特性。整體而言，TP-LCS在保持結構穩定的同時，展現出兼具強度與彈性的特性。有限元素分析與實驗結果趨勢一致，顯示其設計在應力分佈與結構穩定性方面具有一致性與重現性。結論：本研究成功開發出一種核心為「二段式可調控復位加壓螺釘」另外搭配鎖定式螺釘頭及鬆質骨螺紋之新型鎖定螺釘（TP-LCS），其創新結構設計可在維持鎖定穩定性的前提下，提供可調復位與動態加壓能力，有效改善傳統鎖定螺釘於鬆質骨區域固定力不足與剛性過高的問題。此螺釘在有限元素與生物力學層面均展現優越表現，能在 30% 至 90% 接合範圍內維持穩定應力分布，並顯著提升抗拔出力與插入力矩。TP-LCS的提出代表鎖定螺釘設計由「剛性固定」邁向「可調穩定」的新世代，兼具臨床實用性與生物力學合理性。未來將持續進行動態加壓力分析與軸向剛性測試，並推展至動物實驗與臨床研究，以驗證其長期效益與臨床安全性。本研究建立了從設計、模擬至實驗驗證的完整研究流程，為鬆質骨區骨折固定提供新的工程解決方案與臨床方向。.

520 3 $a Background: The development of locking screw and plate systems has greatly improved fracture fixation stability, particularly in osteoporotic patients, where performance surpasses traditional lag screws. However, clinical experience over the past decade has revealed persistently high failure rates of locking constructs in cancellous bone–dominant regions such as the femoral neck, intertrochanteric area, and proximal humerus. The primary cause lies in excessive construct rigidity, which limits adequate micromotion and compression at the fracture site, leading to delayed union or nonunion. Although semi-rigid designs such as the Dynamic Locking Screw (DLS) and Far Cortical Locking (FCL) have been introduced to enhance stress distribution and callus symmetry, each has limitations: DLS sacrifices overall strength, restricting its use to low-load regions, whereas FCL requires far-cortex engagement and thus applies only to diaphyseal bone. Current locking systems therefore lack balance between rigidity control and compression, failing to provide both stability and flexibility essential for cancellous bone fixation. Purpose: This study aimed to develop a novel series of “two-part adjustable reduction compression screws” that can be equipped with a locking head and cancellous thread configuration, termed the Two-Part Locking Cancellous Screw (TP-LCS). The TP-LCS integrates dual functions of adjustable reduction and controllable compression to address the insufficient fixation strength and excessive rigidity of conventional locking screws in cancellous bone. The design combines the deep anchorage capability of cancellous threads with the angular stability of locking screws, utilizing a two-part telescopic configuration that allows minor post-insertion adjustment and compression, thereby achieving intimate contact and stable reduction at the fracture interface. Through finite element analysis (FEA) and biomechanical testing, the study evaluated its mechanical behavior—including stress distribution, pullout resistance, and torsional properties—and compared results with conventional locking screws to assess feasibility and clinical potential. Methods: The research comprised two parts. First, finite element analysis was conducted using ten three-dimensional TP-LCS models with engagement lengths ranging from 10% to 100% (10% intervals). The screw material was Ti6Al4V alloy (elastic modulus 113.8 GPa, Poisson’s ratio 0.342, yield strength 790 MPa). Each model was subjected to a 1000 N axial pullout load and a 1 Nm bending moment to simulate physiological stress. Maximum von Mises stress and fatigue zones were analyzed to assess structural response. Second, biomechanical tests followed ASTM F543-13 standards, including torsional, insertion/removal torque, and axial pullout tests. Five TP-LCS and five AO/Synthes locking screws were tested in synthetic bone blocks (Sawbones, density 0.32 g/cm³). Data were expressed as mean ± SD and analyzed by independent t-tests and Mann–Whitney U tests, with p < 0.05 considered significant. Results: FEA showed that TP-LCS stress distribution varied with engagement length, with the most uniform pattern at 30%–90%, effectively reducing fatigue concentration and potential failure. When engagement was below 20% or at 100%, stress accumulation increased, suggesting these extremes should be avoided. Biomechanical testing demonstrated that TP-LCS had significantly higher insertion torque and axial pullout strength than conventional screws (p < 0.05), indicating stronger fixation and bone–screw interface stability. Its torsional strength was about 80% of the control—likely due to a smaller core diameter—but still met safety standards and exhibited ductile, non-brittle behavior. Overall, TP-LCS provided both strength and flexibility while maintaining stability. Consistent findings between FEA and mechanical tests confirmed the reliability and reproducibility of the design. Conclusion: This study successfully developed a new Two-Part Locking Cancellous Screw (TP-LCS) that combines an adjustable reduction compression core with a locking head and cancellous thread design. The innovative configuration provides controlled compression and dynamic adjustability while maintaining locking stability, thereby addressing the limitations of conventional locking screws in cancellous bone regions. Both finite element and biomechanical results confirmed superior performance, with uniform stress distribution (30%–90% engagement) and significantly higher pullout and insertion torque. The TP-LCS represents a next-generation design concept—shifting from “rigid fixation” toward “adaptive stability”—balancing mechanical strength with flexibility. Future work will include dynamic compression and axial stiffness analysis, followed by animal and clinical validation, to confirm long-term safety and benefit. This integrated workflow—from design through simulation to validation—offers a novel engineering solution for internal fixation in cancellous bone fractures..
